I need some help here. Ever since I was 12 or 13 I have gradually got numbness and no feelings

head of my penis and around the head in some places. I have had a hernia when I was

It could be mole or freckle on your penis head, I suppose, but I can't think of a connection with your masturbation. Perhaps the other spots are bruises of some sorts from trauma.
