Perhaps he is so concerned about maintaining his erection that the mental "pressure" just causes his penis to become flaccid. Somehow he needs to gain some confidence in himself. That will require a lot of understanding and support from you. Also, in my opinion he should see his doctor. Yes, us guys have a great deal of difficulty talking to a doctor about such issues. But do gently encourage him to see his doctor.
May I suggest that you try something a bit different next time. Enjoy your foreplay as you always do. Let him stimulate your clitoris until you have an orgasm. Then manually/orally stimulate his penis until he ejaculates. Don't even tell him what your plans are. Let that be a surprise. Hopefully when he realizes that he can maintain his erection up through ejaculation that that will help to give him some confidence.
